We have a variety of forms listed below to help you with registration, contracts, choreography requests and more.  Submitting the form sends it directly to our office staff, and we will respond normally within 24 hours during business hours.  The forms listed immediatly below this text are online forms.  Just click the form name and it will take you to another web page to fill out the required information.  All other forms can be downloaded and filled out on the computer, then printed and sent to our facility.

iconCamp participant Medical Release Form

This form is necessary for all students who are not current members of Elite, but wish to participate only in a camp or event held at Elite.  Each participant must print and fill out the form completely, and should bring it to the event they have registered for.  This form most often applies to tumble camps, school camps & clinics, and all star camps & clinics.

This form is designed for those who do not have internet access, or those who need forms for an entire squad or group.  If you are signing up for classes individually, please be sure to use our online class sign up form found under "Classes".  Thanks for your cooperation, and we look forward to helping you reach your goals at Elite Cheer & Tumble.
